Patients are seen by appointment only.
Same day appointments are available for urgent cases.
We will need a referral from your obstetrician before you can be seen.
To schedule an appointment, call 281-338-7693 during regular office hours.
Please call at least 24 hours in advance if you need to reschedule or cancel an appointment.
We will need a photo ID and insurance card.
If you are able to complete our required paperwork on-line you can bring these (new patients).
Please notify us of any changes in insurance, address and contact information.
Please arrive 15 minutes before your appointment.
If you are more than 20 minutes late to your appointment, we reserve the right to reschedule your appointment to a different day or time.
Any pertinent records from past hospitalizations or from other specialists if you have medical or surgical issues.
If you are on medications other than prenatal vitamins, please bring the bottles/prescriptions to your appointment so that the physicians can review them with you.
You are welcome to bring family members, friends and children to the visit, but please keep in mind that the rooms can accommodate only 4 additional people at one time. An adult other than the patient must come to the appointment to watch small children.
You will be required to fill out a HIPPA form, notifying us who can have access to your health information. If you do not want a certain person(s) present while the doctor discusses information with you, please notify the ultrasound technician.
We verify benefits but we encourage you to call your insurance to verify that we are on your insurance plan.
We accept cash, Mastercard, and VISA. Payment is expected at the time of service.
We will submit claims to your primary insurance.
We will call you before your appointment to confirm your appointment date and time and to notify you of any co-pays or past due amounts that will be collected at check in.
Please direct any questions about your bill to your insurance company or to our billing department.
We charge 25.00 dollars for filling out FMLA and other forms for release from work.
If any lab work is done, please be aware that the lab bills separately from us and we have no control over what you may owe the lab.
We attempt to remain on schedule. However, emergencies or unexpected situations arise (birth defects, difficult ultrasounds) that may delay your appointment. Please be patient as we may be dealing with work with these patients and/or their family. We will notify patients if we are running behind.
Dr. Rowe and Dr. Ayad request that no one use cell phones while in the ultrasound room. Cell phones are distracting and we want to devote our attention to you and your baby.
You can purchase a DVD of your ultrasound for 10.00 dollars. Pictures taken during the ultrasound will be sent to your cell phone (for free).
Food and drinks are not allowed in ultrasound or exam rooms. You will be asked to not bring in your food and drinks.
